test(server): add metadata provider chain and plugin init-order tests

F2: 5 tests for provider chain fallthrough (parallel Promise.allSettled), timeout handling, and fuzzy sort correctness. F5: 10 tests for plugin ordering, circular dependency detection, and postcondition verification.

Ultraworked with [Sisyphus](https://github.com/code-yeongyu/oh-my-openagent)
