In PciSioSerialDxe driver, gEfiDevicePathProtocolGuid appears twice in
[Procotols] section. In PiSmmCore driver, gEfiSmmSxDispatch2ProtocolGuid
appears twice in [Procotols] section. In PiSmmIpl driver,
gEfiEventReadyToBootGuid appears twice in [Procotols] section. In
DxeCapsuleLib driver, gEfiCapsuleVendorGuid appears twice in [Guids]
section. The duplicate ones should be removed.

Currently, mSmiManageCallingDepth is incremented always whether SMI
handler is present or not. However get decremented only when SMI handler
is found. This causes mSmiManageCallingDepth to grow infinitely as long
as SMI handler isn't present.
Increment mSmiManageCallingDepth only when SMI handler presence is
confirmed.

Replace traditional `#ifndef`/`#define`/`#endif` include guards with
`#pragma` once.
`#pragma once` is a widely supported preprocessor directive that
prevents header files from being included multiple times. It is
supported by all toolchains used to build edk2: GCC, Clang/LLVM, and
MSVC.
Compared to macro-based include guards, `#pragma once`:
- Eliminates the risk of macro name collisions or copy/paste errors
where two headers inadvertently use the same guard macro.
- Eliminate inconsistency in the way include guard macros are named
(e.g., some files use `__FILE_H__`, others use `FILE_H_`, etc.).
- Reduces boilerplate (three lines replaced by one).
- Avoids polluting the macro namespace with guard symbols.
- Can improve build times as the preprocessor can skip re-opening the
file entirely, rather than re-reading it to find the matching
`#endif` ("multiple-include optimization").
- Note that some compilers may already optimize traditional include
guards, by recognzining the idiomatic pattern.
This change is made acknowledging that overall portability of the
code will technically be reduced, as `#pragma once` is not part of the
C/C++ standards.
However, this is considered acceptable given:
1. edk2 already defines a subset of supported compilers in
BaseTools/Conf/tools_def.template, all of which have supported
`#pragma once` for over two decades.
2. There have been concerns raised to the project about inconsistent
include guard naming and potential macro collisions.
Approximate compiler support dates:
- MSVC: Supported since Visual C++ 4.2 (1996)
- GCC: Supported since 3.4 (2004)

The various cores all attempt to print the EfiFileName when
loading/dispatching drivers, but they are not unified on
approach. This commit ensures they are using the same buffer
size and the loop parsing variables are unsigned, as we should
not have a negative index.

Today, DXE/PEI/SMM Core's image loaders only print driver load messages
if debug code is enabled. However, these are some of the most
important prints in the codebase: on a given system even if you
have nothing else to debug with, you can see the last driver
executed.
Debug code blocks are used to skip logic that only exists for
debug purposes and wastes time on a release build. However, the
logic to print a line and determine the filename from the PDB
is not extensive and provides critical information, so it is
inappropriate to wrap in a debug code section.
Platforms can still choose to disable logging at DEBUG_INFO/DEBUG_LOAD
and will not see the error messages.

With current implemenation, all 3 SmmCommunication* functions go through
the same routine, which will dereference the incoming pointer to inspect
whether this is a V3 buffer or not.
However, the caller always pass in the physical addresses, which could
cause the system to page fault after OS take over the runtime control.
This change reverted the common routine to its previous form to handle MM
communicate v1 and v2. Additionally, a specific communicate function for
v3 was created to support MM communicate v3.

MM communicate protocols are expanded with EFI_MM_COMMUNICATE_HEADER_V3
structure that cooperates with updated field types and flexible array.
The PiSmmCore implementation is updated to detect and process incoming
data accordingly.
Two checks are also performed to prevent legacy communicate data or
unsupported data is fed into MM core under agreed header guid.

This patch fix a use-after-free issue where unregistering an
SMI handler could lead to the deletion of the SMI_HANDLER while it is
still in use by SmiManage(). The fix involves modifying
SmiHandlerUnRegister() to detect whether it is being called from
within the SmiManage() stack. If so, the removal of the SMI_HANDLER
is deferred until SmiManage() has finished executing.
Additionally, due to the possibility of recursive SmiManage() calls,
the unregistration and subsequent removal of the SMI_HANDLER are
ensured to occur only after the outermost SmiManage() invocation has
completed.

In last patch, we add code support to unregister SMI handler inside
itself. However, the code doesn't support unregister SMI handler
insider other SMI handler. While this is not a must-have usage.

In UnsetGuardPage(), before SmmReadyToLock, remove NX and RO
memory attribute protection for guarded page since
EfiConventionalMemory in SMRAM is RW and executable before
SmmReadyToLock. If UnsetGuardPage() happens after SmmReadyToLock,
then apply EFI_MEMORY_XP to the guarded page to make sure
EfiConventionalMemory in SMRAM is NX since EfiConventionalMemory
in SMRAM is marked as NX in PiSmmCpuDxe driver when SmmReadyToLock.

Current free pool routine from PiSmmCore will inspect memory guard status
for target buffer without considering pool headers. This could lead to
`IsMemoryGuarded` function to return incorrect results.
In that sense, allocating a 0 sized pool could cause an allocated buffer
directly points into a guard page, which is legal. However, trying to
free this pool will cause the routine changed in this commit to read XP
pages, which leads to page fault.
This change will inspect memory guarded with pool headers. This can avoid
errors when a pool content happens to be on a page boundary.

The MM communicate 2 protocol was introduced to factor out the mismatch
between traditional MM, which requires the physical address of the MM
buffer to be passed, and standalone MM, which copies the MM communicate
buffer data into a separate buffer, requiring the virtual address. For
this reason, MM communicate 2 carries both addresses, allowing the
implementation to decide which address it needs.
This hides this implementation detail from the callers of the protocol,
which simply passes both addresses without having to reason about what the
implementation of the protocol actually needs.
Note that the old version of the protocol is retained, in order to support
existing implementations that don't require this flexibility.

In the CoreStartImage() function [MdeModulePkg/Core/Dxe/Image/Image.c], if
the image entry point returns a failure code, then the DXE Core logs a
helpful DEBUG_ERROR message, with the following format string:
"Error: Image at %11p start failed: %r\n"

The SmiHandlerRegister() function explicitly casts "SmiHandler" (of type
(SMI_HANDLER*)) to EFI_HANDLE, when outputting "DispatchHandle".
Apply the same cast in the counterpart function SmiHandlerUnRegister(),
which compares multiple "SmiHandler"s against the input "DispatchHandle".
This is a semantic cleanup; there is no functional change.

Update the data structure signatures in the PiSmmCore
to be unique. Some of the signatures are the same as those
used by the DxeCore. For example, the memory pool head and
tail structures use the same signature in the PiSmmCore
and DxeCore. This means the use of the wrong FreePool API
will not be noticed at the time FreePool is called and a
failure can occur much later. By making the signatures
unique, misuse of an API is caught at that time the API is
called.

Today's PiSmmIpl implementation initially sets SMRAM to WB to speed
up the SMM core/modules loading before SMM CPU driver runs.
When SMM CPU driver runs, PiSmmIpl resets the SMRAM to UC. It's done
in SmmIplDxeDispatchEventNotify(). COMM_BUFFER_SMM_DISPATCH_RESTART
is returned from SMM core that SMM CPU driver is just dispatched.
Since now the SMRR is widely used to control the SMRAM cache setting.
It's not needed to reset the SMRAM to UC anymore.

Call BS.AllocatePages in DXE driver and call SMM FreePages with the address of the buffer allocated in the DXE driver. SMM FreePages success and add a non-SMRAM range into SMM heap list. This is not an expected behavior. SMM FreePages should return error for this case and not free the pages.
